Common Uses of Healthcare Information
The most common use or disclosure of your healthcare information is for our healthcare records, insurance claims, or other healthcare correspondence. Examples of how we use or disclose information include the following:

● Booking an eye exam appointment
● Electronic medical records of your examination
● Prescribing of glasses and/or contact lenses
● Sending a request for prescription medication to the pharmacy
● Outside referrals to another doctor’s office
● Request of medical records from a previous doctor’s office
● Preparing or sending bills and insurance claims
● Collecting of unpaid bills
● Insurance audits

Other Uses of Healthcare Information
There may be situations where law requires Opt Vision, Ltd. to use or disclose your health information without your informed consent. Such situations may include but are not limited to the following:

● Audits by Medicare, Medicaid, or other insurance providers.
● Disclosures for judicial and administrative proceedings, such as in response to subpoenas or order of courts or administrative agencies.
● Disclosures for law enforcement purposes, such as to provide information about someone who is or is suspected to be a victim of a crime, to provide information about a crime at our office, or to report a crime that happened somewhere else.
● Disclosure to a medical examiner or to organizations to handle organ or tissue donations.
● Disclosures related to worker’s compensation programs.
● Disclosures to business associates who perform health care operations for us such as medical billing services.
● Unless specifically requested, we will also share information about your health care with a family member who may be aiding or translating for you during an eye exam.

Authorization for Other Uses
For other uses of your healthcare information that are not directly outlined in this policy, you may provide our office with a written authorization to release your information.

Notice of Privacy Policy
By law, OPT Vision, Ltd. vow to abide by the terms or this Notice of Privacy Policy until it is changed. OPT Vision, Ltd. reserves the right to change this notice at any time as allowed by law. If OPT Vision, Ltd. changes the Notice, the new policy will apply to all healthcare information, past and present.
